What To bring with you
on your first visit
- Valid photo ID (driver’s license, state ID, or passport)
- Dental insurance card (if applicable); both sides
- List of current medications including dosages, vitamins, and supplements
- Medical history information including any recent hospitalizations or surgeries
- Previous dental records or X-rays (if available from another dentist)
- List of questions or concerns you’d like to discuss
